Application for a Medicare Entitlement Statement (MS015)
This form is used to apply for a statement showing you were not eligible for Medicare during a financial year, which may exempt you from paying the Medicare levy.

- Copy of your current visa (if applicable)
- Letter from Department of Home Affairs about permanent residency application status (if applicable)
- European Health Insurance Card or other proof of health insurance (for certain countries)
