Medicare Claim (MS014)
Services Australia form for claiming Medicare benefits by mail. For paid and unpaid accounts, individual claims, or claims on behalf of a patient.
When do you need this form?
When you've paid for a medical or dental service out of pocket and want to claim back your Medicare benefit by post. Most people claim online through myGov, but this form is useful if you don't have a myGov account or your provider hasn't set up electronic claiming.
What you'll need
- Your Medicare card number and reference number
- Original itemised invoices for each service you're claiming
- Proof of payment (receipts) if you've already paid in full
- Your BSB and account number for the EFT payment
Where to send it
Post the completed form along with your original invoices and proof of payment to: Services Australia, Medicare, GPO Box 9822, in your capital city.
