Application for a Medicare provider number and/or prescriber number for a nurse practitioner or midwife (HW088)
This form allows eligible nurse practitioners and midwives to apply for Medicare provider numbers and/or prescriber numbers to provide Medicare services and prescribe medications.
When do you need this form?
You need this form if you are a nurse practitioner or midwife who wants to provide Medicare services, refer patients to specialists, or prescribe medications under the PBS at a specific practice location. You must use this form if you're applying for your initial provider number, cannot apply online, or are applying for a prescriber number separately from your Medicare provider number.
What you'll need
- Current registration with the Nursing and Midwifery Board of Australia
- Endorsement as a nurse practitioner or midwife with scheduled medicines endorsement
- Approved prescribing qualification (if applying for prescriber number)
- Details of the practice location where you will provide services
