When do you need this form?
You need this form if you are a current or former member of the Australian Defence Force who requires treatment for a mental health condition and want DVA to cover the costs. This includes permanent force members with continuous full-time service, and reservists who served in Border Protection Service, Disaster Relief Service, or were involved in a Serious Training Accident.
What you'll need
- Personal details including full name, date of birth, and contact information
- Service details including unit/branch, service number (PMKeyS), enlistment and discharge dates
- Details of overseas service or specific operations (if applicable)
- Banking details for payment purposes
- Proof of identity documents (if DVA doesn't already have them on file)
