Information Access Application Form (D8601)

This form is used to request access to information and documents held by the Department of Veterans' Affairs.

When do you need this form?

You need this form when you want to access personal information, service records, or other documents that DVA holds about a veteran or their claims. This could be for yourself as a veteran, or on behalf of another person such as a family member or as their advocate.

What you'll need

  • Personal details of the applicant including contact information
  • Details of the DVA client whose information is being requested (if different from applicant)
  • DVA file number and service details if known
  • Proof of identity and relationship to the client if requesting someone else's information
  • Written authority from the client if acting on their behalf

Submit the completed form to DVA's Information Access Unit by email at information.access@dva.gov.au, by post to GPO Box 9998 Brisbane QLD 4001, or by calling 1800 838 372.
