This form provides detailed information about a specific injury or disease when claiming rehabilitation, compensation, or liability from the Department of Veterans' Affairs.
When do you need this form?
You need this form when you're a veteran or defence member making a claim for an injury or disease and need to provide detailed information about each specific condition. This form must be attached to either a D2020 Claim for Rehabilitation and Compensation or D2051 Claim for Liability and/or Reassessment of Compensation.
What you'll need
- Details of your injury or disease including signs and symptoms
