This form is used to claim reimbursement for travel expenses incurred when attending review hearings or obtaining medical evidence for Veterans' Review Board, Administrative Appeals Tribunal, or Specialist Medical Review Council proceedings.
When do you need this form?
You need this form when you have traveled to attend a hearing at the Veterans' Review Board, Administrative Appeals Tribunal, or Specialist Medical Review Council, or when you traveled to obtain medical evidence for your review case. You must submit your claim within 12 months of completing your travel to be eligible for reimbursement.

- Travel receipts (for expenses $30 or over)
- Details of appointment dates and times
- Distance traveled and transport costs
- Accommodation receipts if applicable
- Unused travel warrants if any
Where to send it
Send the completed form with all required receipts and documentation to Department of Veterans' Affairs, GPO Box 9998 Brisbane QLD 4001. The form must also be signed by an authorised VRB, AAT or SMRC officer before submission.
