When do you need this form?

When an application for child support assessment has been made for a child, Services Australia requires additional information to verify the parental relationship. This form is needed to legally confirm that you are the parent of the child or children named in the child support application.

What you'll need

  • Your Child Support Customer Reference Number or Centrelink Customer Reference Number
  • Your personal details including full name, date of birth, and address
  • Details of each child including full name, date of birth, and place of birth
  • Access to an authorised person to witness your signature

Where to send it

You can upload this completed and witnessed form through your Child Support online account via myGov, or submit it to Services Australia. The form must be signed in the presence of an authorised person as listed on the form.
